Provider Perinatal Depression Resources

If you are a provider and interested in making a referral to one of the perinatal mental health agencies listed below, select ONE agency, print and complete the Maternal Behavioral Health Referral Form. Options are also available for Commercial Insurance patients.

The CRPN has established guidelines for response time and feedback regarding referrals that you make to one of the perinatal mental health agencies.

 Applewood Centers   

Supportive Options and Resources for New Moms: Behavioral health care services for pregnant women and new moms experiencing depression, anxiety, and stress in Cuyahoga County. Services will be provided in office-based or home settings, covered by Medicaid and select commercial insurance providers, with some out-of-network and self-pay options available.  Services include assessment, counseling, linkage for pharmacological management, case management and care coordination.

Healthy Moms, Happy Families: Behavioral health services for moms-to-be and new moms of all ages experiencing stress and depression, and/or substance use issues. In-home or office-based services include assessment, counseling, medication management, case management and care coordination. Services covered by Medicaid and select commercial insurance providers. Out-of-network and self-pay options available. Office-based services in Shaker Heights and 6001 Woodland Ave, Cleveland.

Signature Health Inc (formerly Connections)

Women’s Program: Offering home or office based specialized mental health care for women who are contemplating pregnancy, pregnant, postpartum, experiencing pregnancy loss, coping with infertility, or struggling with mood swings during the menstrual cycle or during menopause. Serves adults, teens, and children. Offices in Beachwood, Cleveland Heights, and Ohio City. Services available for those with no insurance as well as Medicaid and Medicare.

 Far West Center                      

Help for Mom Program: Office-based counseling and other mental health services for pregnant women and new moms 18 years old or over experiencing stress and depression. Serves Lorain and Cuyahoga County. Offices in Westlake and Amherst. Child care available. In-home intake visit provided as needed. Services covered by Medicaid and select commercial insurance plans. Services available for those who are uninsured. Spanish speaking psychiatrist and case managers available.


Maternal Depression Program: Counseling and mental health services for pregnant and postpartum women and teens experiencing depression, anxiety, and/or substance use issues. Home-based counseling available in Cuyahoga, Lake, Lorain, Summit, Stark, Portage, and Franklin Counties. Office-based services in Euclid, Garfield Heights, Brook Park, Lakewood, Cleveland, and Lorain.
